Care Credit Payment Plan Instructions

Step 1: Apply

• Click on the link below to access the Care Credit Application: https://www.carecredit.com/go/857PHK/

• Click "Prequalify" and go through the application process​

• If it asks you for an "Invoice Number", you can simply put today's date (numbers only. For examples 09122026)

 

Step 2: Make Payment

• Click "Pay" to pay for the anesthesia service

Step 3: Let us know you paid

• Email office@thedreampros.com and let us know that you've completed the payment process

• Include

  1. Patient's name

  2. Payer's name (if different, such as a parent paying for a pediatric patient)

  3. Appointment date
